Le tecnologie avanzate e la valutazione funzionale/molecolare in chirurgia ortognatodontica

L’obiettivo principale che ci prefiggiamo di raggiungere con questo studio è:

-       determinare i vantaggi dell’introduzione di nuove tecnologie in chirurgia ortognatica in termini di: accuratezza e predicibilità del risultato nella fase di planning chirurgico, accuratezza e precisione nella fase chirurgica (riposizionamento mascellare e/o mandibolare realmente come da set-up preoperatorio, minor sanguinamento, minore probabilità di ledere strutture nervose, minore traumatismo su denti e mucose), rapidità di recupero nell’immediato post-op,  accuratezza/ obbiettività di giudizio/ riproducubilità di osservazione nella fase di valutazione degli outcomes (funzionali ed estetici sui tessuti duri e molli).

Poiché i cambiamenti sulla dentizione dopo ortochirurgia sono drammatici e conoscere i processi di adattamento del controllo motorio della funzione masticatoria dopo chirurgia è di grande interesse sia per i dentisti/ortodontisti che per i chirurghi, ulteriori obiettivi che ci prefiggiamo di raggiungere con il nostro studio sono:

-       determinare la funzionalità dell’apparato stomatognatico e della postura del soma prima e dopo trattamento ortochirurgico per i singoli pazienti con malocclusione dento-scheletrica inclusi nello studio;

-       determinare se vi sia una correlazione tra tipo di malocclusione (II° classe dento-scheletrica, III° classe, open bite) e un determinato pattern di funzionalità masticatoria e/o postura pre e post intervento. Per raggiungere questo obiettivo i pazienti verranno inclusi in gruppi e sottogruppi sulla base del tipo di malocclusione di cui sono portatori. Sarà valutato pertanto l’andamento del gruppo-sottogruppo e non più del singolo paziente, come invece per l’obiettivo precedente;

-       determinare i livelli di espressione delle integrine e di altre proteine (es i sarcoglicani) nel m massetere bilateralmente dei pazienti inclusi nello studio. Valutare se ci sono differenze di espressione tra lato destro e sinistro per i singoli pazienti e all’interno di gruppi-sottogruppi selezionati sulla base del tipo di malocclusione.

Research activities

- Tecnologie avanzate in chirurgia ortognatodontica (tema del dottorato di ricerca).

- Altri campi di interesse nell'ambito della chirurgia Maxillo-Facciale:

malformativa cranio-facciale, chirurgia oncologica testa-collo, traumatologia, sindrome delle apnee ostruttive del sonno.

Leibinger Prize (presented by Stryker). Premio assegnato il 23 Settembre 2014 dall'European Association of Cranio-Maxillo-Facial-Surgeons (EACMFS).

Motivazione: premio assegnato ad un giovane chirurgo Maxillo-Facciale per il miglior curriculum presentato al XXII Congresso dell'EACMFS-Praga 2014.

Il vincitore trascorrerà un periodo di formazione/istruzione c/o un prestigioso Centro di Ricerca/reparto di Chirurgia Maxillo-Facciale riconosciuto dall’EACMFS.

NEW TECHNOLOGIES, FUNCTIONAL AND MOLECULAR EVALUATIONS IN ORTHOGNATHIC SURGERY

BACKGROUND:

To date, there is a strong disagreement about the effects of orthognathic surgery on the stomatognathic system and body posture.

STUDY DESIGN

Introduction: The aim was to determine the characteristics and changes of the stomatognathic system function and of the head/body posture in patients treated for severe dental-skeletal malocclusion.

Materials and Methods: Twenty-one patients with severe maxillofacial deformities were recruited and analysed before and after orthognathic surgery. Patients underwent the following protocol before and after surgical correction: 1) clinical, gnathological and surgical evaluation, 2) stone casts, 3) digital photographs, 4) 3D surface laser scanner, 5) X-ray evaluation (Panoramic X-ray, lateral and frontal tele X-ray, 6) lateral and frontal cephalometry evaluation, 7) chewing cycles.

Biopsies were obtained under general anesthesia from the superficial and anterior portion of both masseter muscles of patients undergoing orthognathic surgery to reposition one or both jaws. The biopsy specimens of both masseter muscles were analyzed using immunohistochemical analysis.

Results: Preliminary results showed an improvement of occlusion, joint and muscle function and posture. Besides, our results showed that integrins in masseter muscle play a key role in regulating muscular func­tional activity and they also allow the optimization of the contractile forces of the muscle.

Future goals of research:

  1. to enlarge the sample;
  2. to compare a mean 3D facial surface laser scan before surgery (T0) with a mean scan obtained after surgery (T1) and also to compare T0 and T1 with prototype of virtual optimal face M (for male) and F (for female);
  3. to study the characteristics and changes of the stomatognathic system function and of the head/body posture on the basis of the specific type of dental-facial deformities (main groups: Angle Class I, II, III; Sub-groups: open-bite, deep-bite, mono cross-bite, bilateral cross bite);
  4. to understand the differences of the protein composition and structural arrange­ment of masseter muscle fibers in respect to other muscle fibers. Thus, it is intriguing to examine whether other proteins, such as sarcoglycans, are involved in this different protein arrangement of masseter muscle fibers.
